Donors: Please fill out the Donor History Packet

Purchase and Storage Clients (buying or storing donor specimens)
Please fill out the Patient Information, P1, P2 and P3 forms

	P1 = AUTHORIZATION FOR RELEASE OF SEMEN
	(Note:  Be sure to have your physician fill out and sign form P1)
	P2 = FROZEN SEMEN SPECIMEN AGREEMENT
	P3 = FROZEN DONOR SEMEN SPECIMEN AGREEMENT

When these have been filled-out, mail or fax them to our office. You can also e-mail them as an attachment.

Storage Clients: (Patients wanting to self-store specimens):
Please fill out the Patient Information Form and the Specimen Storage Agreement
Donor Insemination Patients: Please remember to fill out the Outcome of Insemination Report to let us know how you are doing.
